Account Creation Attestation

The purpose of this Investigator Sponsored Study (ISR), Grant, and Sponsorship portal is to allow physicians and other health care professionals to submit proposals and background information to Sirtex. To request support or submit proposals, you must register with this portal.

This portal will allow you to establish a personalized account that allows you to access your saved and submitted applications at any time. The account will also allow us to send you a submission confirmation e-mail, notify you if additional information is necessary to process your application, and send e-mail updates about any submissions. Once you have completed your registration, you will receive an email asking to verify your account. Use the verification code provided to log into the Sirtex portal and submit your application.

To create a personalized account, you must provide some personal information, including your name and email address. The information you provide to create an account on this portal and to submit a proposal through this portal will be stored and used to manage your account, conduct business, and to contact you about your account and/or submission.

Your personal information provided to create an account and submit an ISR proposal may be shared with other Sirtex affiliates, regulatory authorities (in the United States and outside the United States), as well as Sirtex vendors who are required to keep such information confidential. When you submit personal information via this portal, you do so through the secured portion of our web site. Firewalls and other security technology are used to prevent unauthorized access to your account information. The Sirtex Privacy Policy further explains how your personal information is handled.

By clicking the 'Create Account' button you hereby acknowledge and agree to the following terms:

• You acknowledge and agree that applicable law may require Sirtex or its affiliates to report (disclose) information on monetary or non-monetary support or other remuneration provided to physicians and other health care professionals to government agencies and/or the public, and that once reported this information may be publicly accessible. Specifically, information regarding funding or other support provided to you, your identity, the monetary value of the funding or other support and other information that Sirtex determines, in good faith, is required may be publicly disclosed. In addition, you acknowledge that the information you provide may also be subject to internal (within Sirtex and its affiliates) audits as well as external audits and/or requests.
• You are confirming that you have reviewed, understand, and agree to the Sirtex Privacy Policy and the details noted herein, and you hereby consent and authorize the collection, use, storage and transfer of your personal information for the purposes of using this portal as described herein.
• You agree no confidential relationship regarding any proposal is established or implied by Sirtex's acceptance or consideration of the submitted proposal.
• You hereby authorize and consent to the transfer and processing of the information submitted by you on this portal, including your personal information, to the United States and if applicable other countries outside of your country of residence, where the level of protection may be lower than in your home country.
• You acknowledge and consent and shall cause all study staff to acknowledge and consent in accordance with applicable data protection laws, to Sirtex’s collection, use, processing, and disclosure of Investigator’s and any sub-investigator’s personal data, including details of his/her name, address, qualifications, and clinical trials experience as set out further in the Sirtex Privacy Notice. Additional uses or disclosures may include financial information (including compensation and reimbursement payments), assessments by Sirtex of your and study staff’s suitability for any proposal, and for compliance with applicable law. You understand and agree and shall cause all study staff to agree that this information may, as necessary for the purposes, be made available to European Commission, government authorities and other Sirtex companies or authorized agents, and may be transmitted outside the country of origin as required.
• You acknowledge, and hereby authorize and consent that the information you submit on this portal, including personal information, may be securely retained by Sirtex in its libraries, files and/or on its web sites, and thus may be subject to continuing use and disclosure for the purposes described herein for a duration of up to 50 years from the date of your submission under this account, unless you revoke (cancel or withdraw) your authorization sooner.
• You acknowledge that you have the right at any time, in accordance with applicable laws, to withdraw your authorization to use your personal information by contacting privacy@sirtex.com.
